What are Vantage Health Plan's Disease Management Programs?

Vantage Health Plan's Disease Management Programs are programs that help members to better understand and manage their disease state of Diabetes or Congestive Heart Failure. The programs are offered to qualifying Vantage Health Plan members free of charge and are conducted over the phone with a pharmacist at the member's convenience. As part of the program, members will receive a medication review, health reminders about needed labs / exams, and educational material related to their disease state.

The benefits of participating in the Diabetes Disease Management Program:

Members participating in the Diabetes Disease Management Program will learn about Diabetes, the labs and exams needed each year, as well as details about their medications and how they work to help with diabetes. One important lab to have done every 3 - 6 months is the Hemoglobin A1c. This lab value tells you your average blood sugar over the last 3 months and helps you to know whether or not your diabetes is well controlled. It is also important to have a dilated eye exam every year. Dilated eye exams helps to detect early signs of diabetic retinopathy even before symptoms are present. As part of the program, we also discuss blood sugar monitoring including the best times of the day to check your blood sugar as well as what the value should be.

The benefits of participating in the Heart Failure Disease Management Program:

The Heart Failure Disease Management Program focuses on educating members about their condition, the importance of taking their medications, and signs and symptoms of heart failure. We discuss ways to assess your health daily to determine if symptoms are under control or worsening. Weighing each morning before eating or drinking helps you monitor your fluid status and is highly encouraged. EKG and echocardiogram are two tests used to assess how well the heart is functioning. You should receive an EKG at least yearly, and an echocardiogram should be done if there has been a change in your health. Diet and exercise are discussed as it is important for heart failure patients to eat a low sodium diet and exercise as tolerated. Quarterly educational material is sent to all members thru the mail as well.
